In Germany, about one million people suffer from glaucoma. Another five million are at risk of contracting it.
The clinical picture describes a group of eye diseases that, in the advanced stage, damage the optic nerve and ultimately the nerve cells of the retina. About 10% of glaucoma patients are at risk of blindness if left untreated. In such a case, glaucoma surgery should definitely be considered.
The symptoms of cataracts are the result of too much pressure in the eye. Everyone has a clear fluid in the part of their eye that is not visible from the outside, the so-called aqueous humor.
The eye produces this fluid, which after some time flows out of the eye in uniform quantities via the so-called Schlemm’s canal. The aqueous humor nourishes and stabilizes the eye and is indispensable for our vision.
Similar to the air in a ball, aqueous humor creates a certain amount of intraocular pressure. The most common cause of increased pressure is the damaged or inadequate drainage of aqueous humor from the eye.
If the intraocular pressure is permanently too high, this damages the optic nerve and leads to failures in the visual field. Slow blindness can be the result. The opposite, however, is a glaucoma attack that must be treated immediately. Goniotomy can be used to prevent blindness and maintain the ability to see.


What is treacherous is that the disease develops without symptoms and gradually. The visual field also deteriorates gradually and often unnoticed. Regular preventive care with an ophthalmologist alone can clarify whether there is a risk and which therapy for cataracts makes sense.
Risk factors for a disease are often a higher age, a genetic burden due to pre-existing conditions in the family, higher myopia, uneven corneal thickness and underlying diseases such as high blood pressure. Snoring or migraines can also indicate glaucoma.
For proper glaucoma diagnostics, we rely in our practice on high-quality ZEISS devices that support us with state-of-the-art diagnostics, including the Cirrus™ HD-OCT with extended glaucoma applications, the Humphrey®perimeter and fundus photography using the Zeiss Clarus 500®.
Early detection of the green cataract is important, as already detectable failures in the visual field are no longer curable. On the other hand, timely treatment with eye drops can prevent glaucoma from worsening.
However, it may happen that the pressure-lowering effect of the glaucoma drops is not sufficient. Occasionally, there are also side effects. In these cases, surgical intervention can prevent further damage.

This laser method ensures that the aqueous humor can drain better by activating pigments. This lowers the intraocular pressure for a longer period of time.
Since the procedure is minimal and we perform it within a few minutes in the practice, complications are almost impossible. If necessary, we can repeat this pain-free laser treatment as many times as necessary.
During the procedure called Ab-Interno Goniotomy, the trabecular meshwork, a part of the angle of the chamber of the eye, is removed over a short distance with a very fine knife (KAHOOK knife).
Through a small cut (approx. 1.5 mm), the high-precision, small knife penetrates Schlemm’s canal, an annular collecting tube in the eye. This allows for better drainage of the aqueous humor, which is responsible for eye pressure. This operation is applicable to certain forms of glaucoma.
In this method, a special laser is used. This causes tiny holes in the iris. Through these holes, the eye water can escape and there is a reduction in pressure inside the eye.
