Clear Lens Exchange – What is a refractive lens exchange?

A refractive lens exchange (also called a clear lens exchange) is an alternative to laser eye surgery and describes the replacement of the body’s own lens. This solution is reserved for people with extremely high refractive error and/or presbyopia, since here usually only the replacement of the body’s own lens by an artificial lens can correct the refractive error. 

After a thorough examination and measurement by your ophthalmologist, an artificial lens tailored to your needs will be used for this purpose. Thanks to modern multifocal lenses, presbyopia can also be remedied with this procedure. This is especially appreciated by our patients with progressive spectacles. They rate their experience with multifocal lenses as consistently positive, because with a lens exchange, progressive lenses and reading glasses are a thing of the past.

Since the eye loses its natural ability to adjust itself to different distances (accommodation) when replacing the lens with the implanted multifocal lens, this method is only used to correct defective vision when the ability to accommodate is reduced (approx. from the age of 45) or when the lens is already clouded (incipient cataract).

What are the options for an artificial lens?

When choosing an artificial lens, you have a variety of options: For example, in addition to multifocal lenses, also known as “multi-focal lenses”, there are monofocal lenses or “single-focal lenses”.

While monofocal lenses usually only allow sharp vision at a distance, multifocal lenses represent absolute independence from glasses. Especially people with a high degree of refractive error in connection with presbyopia benefit from this method. 

The toric lens compensates for a corneal curvature. The multifocal toric lens compensates for both nearsightedness, farsightedness or presbyopia as well as corneal curvature.

EDOF lenses use a new technology that has recently emerged in the treatment of presbyopia-correcting intraocular lenses (IOLs). Unlike multifocal intraocular lenses (IOLs) used to treat presbyopia, EDOF lenses work by creating a single elongated focal point to improve the “area of vision” or “depth of field.”

What is the procedure for replacement?

The procedure is performed on an outpatient basis. With the help of eye drops, the eyeball is locally numbed. The incision on the eye closes by itself and therefore does not require a seam. The insertion of a multifocal lens is done in two simple steps: 

Step 1

The body’s own lens is removed: For this purpose, a millimetre fine incision is made (by hand or optionally even more precisely by the new LENSAR laser), after which the lens capsule is opened. The lens core is crushed by means of ultrasound and sucked off.

Step 2

Now the multifocal lens is pushed through this tiny opening and positioned. A small, folded artificial lens made of soft material is inserted into the now empty capsule bag. The lens unfolds gently inside the eye, where it occupies the position of the original, natural lens.

FAQs – Refractive Lens Exchange/ Clear Lens Exchange

A clear lens exchange (CLE), also called a refractive lens exchange (RLE), is an operative exchange of the body’s own lens, usually with the aim of correcting the dioptres. For this purpose, analogous to cataract surgery, the body’s own lens is replaced by an artificial lens.

Especially in the case of particularly severe refractive errors, where laser treatment is not possible, the Clear Lens Exchange can be a sensible alternative to finally be able to see clearly again without glasses or contact lenses.

With Clear Lens Exchange, also known as Clear Lens Extraction, the patient’s own lens is removed and an artificial lens is inserted, which enables sharp vision.

In younger patients, we recommend an ICL lens implant if laser treatment is not possible. In this procedure, the lens is not replaced, but an artificial lens is implanted in front of the clear endogenous lens.

Please note that the ambulatory Clear Lens Exchange is a surgical procedure and you are not allowed to operate a car or other machines and equipment for 24 hours afterwards.

In the following days, sports activities and heavy physical work should be avoided. A visit to the swimming pool or wearing make-up should also be avoided if possible.

Special eye drops or ointments can speed up the healing process when used regularly. In addition, a visual improvement is already noticeable on the following day of the operation. However, the complete final result can only be assessed after several weeks.
