How does cataract develop?

In the case of cataracts, a clouding of the eye lens takes place (so-called lens clouding), which is often the result of the natural aging process, but can also be triggered by certain metabolic diseases. Vision deteriorates significantly.

Cataract symptoms are often manifested by creeping poorer vision, fainter colors, increased glare sensitivity or a veiled, foggy and greyish vision.

If left untreated, cataracts can lead to blindness. The only permanent treatment option is cataract surgery, which is now a routine procedure and can restore your vision.

The successful methods for treating cataracts are manual lens replacement (using a scalpel) and laser-assisted lens replacement. Cataract surgery using modern laser technology is an established method in ophthalmology that is painless, simple and safe thanks to advanced technology. 

Every year, more than 750,000 procedures are performed in Germany to treat cataracts, which has made this operation routine.

In cataract surgery, the clouded lens is replaced painlessly with an artificial lens. The operation is usually performed under local anaesthesia, but general anaesthesia is possible on request.

Cost of cataract surgery

The cost of cataract surgery depends largely on the method chosen and the type of lens. The statutory health insurance (GKV) covers the basic care (surgery with scalpel and standard lens), which corresponds to a share of approx. €465.52 for medical services and approx. €137.00 for the lens per eye.

The following amounts represent the remaining own share for both eyes after deduction of the cash benefits. Approximately €600 in anaesthesia costs must also be borne for the operation.

  • Premium lens package: from approx. €3,582.79 for both eyes. This package includes the preliminary examination, surgery with high-quality special lenses, follow-up care and light pulse therapies.
  • Premium laser package: from approx. €3,992.58 for both eyes. The high-precision femtosecond laser is used for this.
  • Platinum package: approx. €7,122.47 for both eyes. This is the most comprehensive care including femtosecond lasers, premium lenses, aftercare and light pulse therapies.

Sequence of cataract surgery

The procedure of cataract surgery is as follows: In order to be able to expect the best surgical result, we carry out precise preliminary examinations. The course of your cataract disease is accurately diagnosed and the cataract surgery is planned.

You can already carry out the aptitude test online to determine whether you are suitable for surgery. Furthermore, optical biometrics are often carried out using laser technology (IOL master) for lens calculation. The opacity of the lens by means of the slit lamp is also examined. We also check your intraocular pressure.

The LENSAR laser (a femtosecond laser) replaces the hand of the surgeon/ophthalmologist and thus ensures an exactly circular incision on the lens capsule during the procedure of cataract surgery. At the same time, the laser produces perfect access cuts and takes care of the gentle fragmentation of the lens affected by cataracts.

This procedure during cataract surgery only takes a minute and is much safer than the conventional procedure thanks to the LENSAR technology.

The body’s own cloudy lens of the operated eye, which is affected by cataracts, is removed: For this purpose, the old lens core is aspirated.

Now the selected multifocal lens is pushed through the tiny opening of the laser cut and positioned. The new lens must have exactly the same refractive power as the body’s own lens. The multifocal lenses unfold gently inside the eye, which is affected by cataracts, where the lens is anchored with the help of small elastic temples.

The incision during the procedure through which the lens is inserted is so tiny that it does not have to be closed by a seam.

There will be follow-up examinations by your ophthalmologist who will monitor the healing process and accompany you with questions.

An occasional side effect when using an artificial lens is the so-called “secondary cataract” – a slight clouding of the capsule bag – but this is easy and quick to treat.

How long does the healing process take after cataract surgery?

The healing process after cataract surgery usually takes two weeks. The cataract surgery on the eye is usually done on an outpatient basis, a stay in an eye clinic is not necessary.

If both eyes are affected by cataracts, cataract surgery will be performed by our ophthalmologist at intervals of one week per eye.

The lenses are replaced in two operations on the patient.

Immediately after cataract surgery and in the first few days, people should refrain from driving until the ophthalmologist gives the green light again and the patient can see and read well on their own. Antibiotic eye drops are prescribed to support the healing process and prevent inflammation.

Thanks to the possibilities of surgery for cataracts and state-of-the-art surgical methods, people no longer have to go blind with cataracts. On the contrary, full visual acuity when seeing and reading is restored through a new eye lens. Vision aids such as reading glasses become superfluous.

When is the right time to operate on cataracts?

When a cataract is operated on depends on various factors. This primarily includes the subjective sensation of visual impairment.

In some professions and also when driving, a certain visual performance is a mandatory prerequisite. In such cases, surgery is necessary at an early stage. This also applies if the visual performance is not yet severely impaired from a subjective point of view.

A congenital cataract should be operated on immediately after diagnosis. Only then will children have the chance to learn to see correctly.

Basically, you can do anything after cataract surgery. However, there are a few things to keep in mind in the first days after the treatment: On the first day of the operation, you should not operate any heavy equipment/machine and should not drive a car. Avoid pressure on the eye and refrain from strenuous activities such as heavy lifting.

When cataracts should be operated on at the latest depends on the restriction of the person concerned. If the cataract limits your vision, you have difficulty reading and generally notice that you can no longer see well, you should consider a personal consultation for cataract surgery.

A common question is when you can drive again after cataract surgery. In principle, as soon as a vision of at least 70 percent has been achieved again, you can drive again. When this is exactly is, of course, individual. You will receive exact details during your follow-up after the operation. In most cases, however, it should be possible to drive again a week after the operation.

Gardening should be avoided after cataract surgery. The healing process should not be affected. Your eyes are extremely sensitive and more susceptible to infection after surgery. By working in the garden, dust particles from soil and foliage can quickly get into your eye. Branches, twigs and needles could damage your eye. You should simply take care in the first few days after the operation and not do any heavy physical work. Treat yourself to a break. The cataract surgery is painless. The affected eye is locally anesthetized before the operation, only then is treatment started. This way you will not feel anything during the procedure. The cataract is operated on using a high-precision laser, the Lensar laser. The laser replaces the surgeon’s hand and takes care of the gentle fragmentation of the affected lens. The laser makes a small incision in the cornea and removes the diseased lens. This is replaced by a new, artificial lens. The laser method is considered more precise and gentle than the mechanical version.

A cataract surgery usually takes less than 20 minutes. After the cataract laser surgery, the patients are observed for a few hours. The affected eye is protected by a bandage and the patient can return home after the observation period. The cause of the development of the cataract is signs of aging. At an increasing age (from the age of 50), every second person is affected by cataracts. The body’s own eye lens becomes cloudy and gray. This can also affect both lenses.

Other causes that cause cataract development may include:

  • Diabetes
  • Injury to eye
  • heavy smoking
  • radioactive radiation
  • UV light

These are the most common symptoms that occur with cataracts. From patient to patient, these symptoms may change and turn out differently. In addition to cataracts, some people may also show signs of glaucoma, age-related macular degeneration, or other eye damage. Then it must be clarified which eye disease is responsible for the symptoms. Other eye diseases can include presbyopia, distance vision, nearsightedness or retinal detachment.

Yes, the ophthalmologist or an optician can determine whether you suffer from cataracts. It is advisable to make an annual appointment with the ophthalmologist for preventive care.

If cataracts progress slowly, glasses or contact lenses can help for a while. This can compensate for a mild visual impairment. With rapidly worsening vision, there is no alternative to surgery to be able to see better again.

In rare cases (less than 1 in 100 patients), inflammation of the inside of the eye (endophthalmitis) may occur. Therefore, it is important to follow the instructions of the aftercare. The operation is painless and you will be put on an eye bandage. This will be removed at the next day’s check. Please do not remove the bandage yourself. Slight tearing and scratching of the eyes is normal. Furthermore, in very rare cases, an injury to the iris or the eyeball can occur during surgery, with less than 1 in 100 operations.
